25 Facts About Rheumatoid Arthritis (RA) Everyone Should Know 

 

Rheumatoid arthritis is an illness that’s widely misunderstood. I believe this is partly due to the fact that many confuse it with osteoarthritis (OA). This mix-up leads people to believe rheumatoid arthritis is caused by aging.

Rheumatoid arthritis (RA) is in no way “normal” aging and shouldn’t ever occur at any point in a healthy person’s life!

Rheumatoid arthritis is an autoimmune disease – meaning the body’s immune system is attacking itself – in the case of RA the immune system attacks a person’s joints.

Almost 1.5 million Americans have rheumatoid arthritis and it’s on the rise. Unfortunately, modern medicine doesn’t have much to offer for reversal of this terrible condition. However, functional medicine and full body integrative and holistic approaches have resulted in full recoveries and promising outcomes.

I am a perfect example of this. I was diagnosed with Rheumatoid Arthritis (I was also given many other additional diagnoses – another blog on this later) more than a decade ago. When modern medicine had no answers for me other than prescribing toxic immunosuppressive drugs and steroids, I voluntarily chose not to use those. As a practicing Internal medicine doctor who had a special interest in Rheumatology, I knew all too well, the terrible side effects and long term health consequences of these drugs. I was determined to find other answers!

Against conventional wisdom, I decided to be my own guinea pig and looked for other safer but effective alternatives. I researched and studied the fields of nutrition, and Integrative, Holistic, and Functional Medicine. I trained and became board certified in these, using all that I learnt, first on myself. It was trial and error at that point for me but I eventually healed myself from this crippling disease and from all other diagnoses that conventional medicine had given to my wide range of chronic symptoms!

My health completely recovered, and I no longer carry the clinical or lab diagnosis of RA, nor do I suffer from the pain or the disabling fatigue which is characteristic of this disease. In fact, I feel more energetic and vibrant than I did even in my 20s. I now use the same healing modalities and step-wise approach that worked for me, on my patients to help them in their recovery process.

As with everything else in life, we are all individuals with unique and varied characteristics, so no one size of treatment can fit all. Everyone with RA will have different root cause(s). I help my patients and clients find their particular root causes and then use a very personalized treatment approach that is right for their body.

In an effort to clear the confusion and misunderstanding surrounding rheumatoid arthritis, I’ve compiled 25 facts about rheumatoid arthritis that everyone should know. RA is a condition that can strike any person at any age – and it’s best to know these important facts.

RA is an autoimmune disease while osteoarthritis is caused by erosion of the cartilage and bones due to aging, overuse or obesity. Because these conditions are completely different, so are their treatments and it is important not to confuse the two.

Vitamin D has been linked to a reduced risk for nearly all autoimmune disease, so it’s a good idea to daily get about 20 minutes of sunshine, before applying sunscreen (without burning of course). If you have the ability to test your vitamin D levels, please do so and take supplemental Vitamin D under supervision. In my practice I use different strengths of Vitamin D based on my patient’s levels.

Your Ultimate Hurricane Clean Up Guide – From Your Home to Your Body

 

Now that most of Hurricane Harvey’s and Irma’s waters have subsided and we are left to assess the damage, an important time presents itself. Hurricane clean-up is more than gathering debris and clearing out the mud that remains, it involves protecting ourselves from current and future threats. This most certainly includes invisible killers released by the toxic mold that creeps into water damaged buildings and wreaks havoc on our health.

When we think of hurricane clean-up, many don’t realize the significance mold has on our health. It’s hard to keep this invisible threat at the forefront of our efforts when there’s so much wreckage in our path, but we must.

Mold remediation is possibly the most important of aspect of hurricane clean up.

If mold remediation is not done properly and regularly checked for over the years, you run the risk of making yourself and your family very, very sick. As a Functional Medicine physician who helps people with serious chronic diseases I see this all too often!

Now that you’re safe from the immediate dangers of the storm, I cannot emphasize enough the importance of cleaning up from this hurricane with mold prevention and remediation as priority number one.  

Is mold a new health concern?

Mold has always played a role in human health, however it’s only in the past few decades that we’ve started building houses out of what’s essentially paper. Drywall and wood are ideal hosts for robust mold blooms.

All mold requires moisture to grow and thrive. It’s this combination of perfect host materials and high moisture content due to flooding that makes hurricane damaged building a breeding ground for toxic mold. Unfortunately, the Texas and Florida heat exacerbates this situation.

How mold works

Mold’s role in nature is that of a decomposer. Mold grows on dead organic matter, such as leaves and trees, breaking it down. Mold reproduces through the release of tiny airborne spores. These spores are invisible and can be toxic.

Mold toxins have been linked to a whole host of medical issues including autoimmune diseases, such as rheumatoid arthritis, DNA damage, neurotoxicity, and myelin breakdown which can contribute to Alzheimer’s, Parkinson’s, and multiple sclerosis. In fact, Inhalational Alzheimer’s Disease (IAD) is a subtype of Alzheimer’s caused by biotoxins, which has only recently been identified.  

The mitochondria of our cells evolved from bacteria, and bacteria’s natural enemy is mold and the toxins mold releases. So, it makes sense that certain molds we encounter would carry the unique ability to damage our mitochondria. It’s how we develop antibiotics (think Penicillin). We use mold to create antibiotics responsible for killing harmful bacteria (and sometimes inadvertently we end up also killing the good bacteria).

Properly cleaning up after a hurricane is critical to preventing damaging mold from growing in your home. It may seem daunting to take the extra steps necessary for killing and preventing future mold growth, but your health and the health of your family depends on it.

Symptoms of mold exposure

Mold can move in surprisingly quick after a flood. In fact, it can already be there when you start the initial work on your house, only a couple of days after the storm.

The symptoms of mold exposure include debilitating fatigue and mood swings, so it could be possible to mistake mold exposure symptoms due to the exhaustion as well as the emotionally taxing situation you’re in, but paying  close attention to your new symptoms is the key. The signs and symptoms of mold exposure can include:

  • Chronic fatigue
  • Muscle weakness and pain
  • Anxiety
  • Mood swings
  • Difficulty concentrating
  • Poor memory
  • Chronic headaches
  • Bruising
  • Nosebleeds
  • Vertigo
  • Stiff joints
  • Tingly legs and arms
  • Insatiable thirst
  • Metallic taste
  • New allergies
  • Rheumatoid arthritis
  • Crohn’s disease
  • Hashimoto’s thyroiditis
  • Other Autoimmune disease(s)

The tricky thing about mold is its symptoms present differently for everyone. This is why it is so hard to diagnose early.

To further complicate matters, there’s an actual gene that 25 percent of the population carries which makes them incredibly sensitive to mold. The HLA-DR gene prevents the individual’s body from successfully ridding itself of toxins. Think of the canaries in a coal mine! I should know as I surely am one!

Often, a family doesn’t realize there’s mold in the house until a person with this gene becomes very ill. Whether or not you have the HLA-DR gene, you should thoroughly clean your house using proper mold remediation protocol. Just because you’re not extra sensitive to mold toxins doesn’t mean you aren’t negatively impacted by them, it just might take you longer for you to notice.

Cleaning up after a flood

Cleaning up after a flood should be taken very seriously. There’s a chance mold has already moved into your home. As you rip out drywall and get your home ready to rebuild, it’s safest to assume mold has already moved in and take necessary precautions.

Also, if you are planning on rebuilding, you should have your home tested by a qualified mold inspector. You’ll need an ERMI test done on your house before any remodeling begins or you run the risk of covering up mold growth.

Remember, you are cleaning up to prevent the growth of a substance that can cause you and your loved ones’ serious long term illnesses, and it takes less than a day for mold to start growing. Be vigilant.

Here’s what you should do when cleaning up a flooded building:

  • Wear a Tyvek suit and face mask when cleaning up. You should use either a full-face mask or a half-face mask with goggles. Moldex makes masks specifically for mold clean up.
  • Everything your Tyvek suit touches when you leave the house could become contaminated – this includes your car. Remove your Tyvek suit and throw it away or leave it outside the house each time you leave the contaminated building.
  • If you start feeling ill, tired, crave sugar or are overly emotional while you’re cleaning, you should leave the house immediately and upgrade your protective equipment before returning to work.
  • You’ll need to clean everything with a suitable cleaner (NOT bleach) and follow up with an encapsulant. Encapsulants are cleaning products with fungicides that prevent mold from coming back.

Porous materials, such as wood, are of greater concern when cleaning, but for the sake of simplicity I’m going to recommend you use the same cleaning procedure for all surfaces, and this does NOT include bleach. Bleach won’t kill the mold in porous materials and it will only cause future mold to grow back stronger.

Instead do this:

  1. Clean everything off with a cleaner, such as Borax or Benefect.

A formula I have used in my home in the past when I’ve had minor leaks:

  • 1 gallon hot water
  • ½ cup white vinegar
  • 1 cup Borax

Mix well. Put above mixture in a spray bottle. Add 10-15 drops of Melaleuca essential oil to the spray bottle each time you refill it.  Shake well and spray on affected surfaces.

(Consider diffusing clove, melaleuca, eucalyptus, lemon, and cinnamon essential oils in multiple diffusers throughout your home)

  1. Follow up with an encapsulant, such as Caliwel or Concrobium.

  • Careful not to cross-contaminate. This means anything from your flooded home should not be taken into a clean environment (your car).
  • You’ll need to seriously consider what’s worth putting your health in danger as you go through all the porous items from your flooded home. This means clothing, curtains, books, furniture, bedding, photos and more.

Fabric should be thrown away when possible. If you must keep something, have it dry cleaned. Photos should be laminated or reprinted.

After you’ve gone through this process and before you begin remodeling, you should have an ERMI test done by a mold professional.

You should also have your home (and yourself) tested for mold allergy as well as mold toxins (known as mycotoxins) over the next year or two – possibly longer, especially if vague and troublesome symptoms persist.

Supporting your immune system during flood clean up

While you’re cleaning up a flood damaged home, you should try to support your immune system as best as you can. Your body is going to be exposed to all kinds of chemicals, toxins, and potentially mold, so it will need any extra support you can offer.

The four immune system supporting recommendations I suggest during clean up include:

  1. An immune boosting diet
  2. Reducing your toxic burden
  3. Supplementation
  4. Be good to yourself/stress reduction

First, I recommend an immune boosting diet.

This means a nutrient-dense diet of healthy fats, proteins, and plenty of vegetables.

Avoid sugar and carbs because they promote inflammation and make it harder for your mitochondria and immune system to do their jobs. Instead healthy fats and proteins should be your main source of fuel.

Also, I recommend avoiding alcohol during clean-up because your liver and immune system need to be freed up to detox your body from more important toxins.

Second, reduce your toxic burden.

Outside of home clean-up, consider other sources of toxins you’d benefit from removing from your life. You can reduce toxin burden by eliminating items such as:

  • Harmful cleaning products
  • Toxin-containing personal care products
  • Fish high in mercury and other metals
  • Paints with VOCs
  • Non-stick pans
  • Fried and charred food
  • Artificial sweeteners
  • BPA-containing plastics
  • Metal dental fillings

Yes, some of these are for later but I just want you to have a full resource.

As a side note, candida overgrowth in your body increases your toxic burden too. I often get asked if candida or yeast overgrowth is because of mold, so clarifying here:

If you have symptoms of candida overgrowth, it is not due to inhaling it in your home air.  You may have toxic mold in your house but the air in your home does not typically contain much Candida albicans. However, exposure to mold can seriously weaken the immune system making our body more prone to candida flares within our digestive system (candida is present in small amounts in all of our gastrointestinal systems), which when severe can lead to systemic and widespread symptoms in the body.

In addition, mold exposure can make one more sensitive to all kinds of fungi (candida is a fungus) and mold. Developing an allergy to mold leads to hypersensitivity to candida and starts a viscous cycle of inflammation and immune system dysfunction with far reaching consequences.

  • Magnesium citrate (important for overall health and prevents constipation). During this time it’s super important your body is able to detoxify efficiently, which is why it’s critical you avoid constipation at all costs.
  • If you have 2-3 bowel movements a day already, you can choose a chelated form of Magnesium glycinate instead. Most of us are low in magnesium and it can be a crucial nutrient especially during stressful periods.
  • Selenium (reduces inflammation, supports thyroid function, and reduces free radical stress) You can get selenium from a good quality multivitamin, and also eat a couple of brazil nuts daily.

If you’re working in a water-damaged home or moving back into a water-damaged home, you need to be more vigilant than you’ve ever been before about your health. The problem with mold is it’s can be almost impossible to see, so paying careful attention to your health is your best barometer for mold toxin impact.

I recommend making an appointment with a functional medicine doctor who specializes in chronic disease management, autoimmune disease(s) and mold toxin exposure. Tell your doctor, you and your home (or work) have been impacted by a flood and you want to pay close attention to your mycotoxin levels and any preliminary symptoms of impending autoimmune disease.

Your functional medicine doctor can test you for mycotoxins and help you develop a preventative plan for mold exposure. Even if your home is cleared for mold and your initial tests come back clean, I recommend checking everything again 3, 6, 9 months and a year later.

After the initial year of checks, I recommend health and home checks every six months to once a year at least after that if your home has been flooded. The reason I recommend such frequent checks is because the earlier you catch mold, the more efficiently and effectively it can be dealt with.

Remember, we’re talking about a substance capable of causing autoimmune diseases, such as multiple sclerosis, rheumatoid arthritis, hashimoto’s thyroiditis, DNA damage, neurotoxicity, Alzheimer’s, and other neuro-degenerative diseases.

Mold is not to be taken lightly.

It’s personal for me!

My neighborhood was flooded during Hurricane Harvey. My family and I had a very close call. Although we don’t live in a flood zone, we were suddenly placed under mandatory evacuation orders on the second day of the storm. To evacuate or to stay put was one of the hardest decisions that my husband and I had to make for our family’s safety and well-being.

After a lot of contemplation we decided to move everyone to the second floor of the house along with essential supplies. The reason for this decision was the ill health of my mother who is physically disabled, with early dementia and lives with us. We couldn’t attempt to evacuate in a car with her and risk getting stuck enroute due to road and highway flooding and closures. That would have been worse! So we stayed…

The water came all the way up to the very edge of our home but we did not have any flooding; unfortunately many of our neighbors weren’t as fortunate. Hurricane Harvey has taken a physical and emotional toll on our community. I myself felt like I was dealing with anxiety, sleep issues, and some form of PTSD for many weeks after. I cannot fathom how those who lost everything must feel.

While I continue to send thoughts and prayers their way, and have continued to collect and donate food and supplies to shelters and individual families in need, I do also hope to spread the word about flood clean up and mold toxin exposure so we prevent future illnesses.

Mold clean up and prevention should be at the forefront of our concerns during the recovery process from the devastation that a hurricane leaves behind.

While I write this, my thoughts and prayers are with all those who are still facing the flash flooding and devastation from Hurricane Maria.

Essential Fatty Acids (EFAs) – Essential to Brain Health

I’m often asked what are the few nutrients I think are absolutely imperative and essential for good health especially in this modern world. One of the top nutrients in my list is Omega Essential Fatty Acids. Every patient who comes into my practice gets their Omega 3 levels and the ratio of Omega 3s to other omegas, like 6s, 9s etc tested and an appropriate individualized diet and supplementation (if required) plan created for them.

Decades of study and countless books have gone into exploring Omega-3 and the role it plays in our physical and mental health.

Here are 8 Essential Facts about this ESSENTIAL substance.

1. Omega-3 is called “essential” because it’s necessary for our health, but we cannot make it on our own.

2. One essential Omega-3 fatty acid is a substance called α-linolenic acid (ALA). Our body can use ALA to make two other essential fatty acids: DHA (docosahexaenoic acid) and EPA (eicosapentaenoic acid). However, we don’t make enough to meet daily needs for optimal health and the conversion of ALA to DHA and EPA is not efficient enough in most humans.

If you are deficient in Vitamin Bs of various kinds, Vitamin C, minerals like Magnesium and zinc you will not optimally convert ALA into DHA and EPA.

Have your physician test you for optimal levels of these nutrients and adjust your dietary plan and your supplement schedule to optimize the levels of these key supportive nutrients.

3. Omega-3 is critical for preventing symptoms of chronic illness, such as inflammation, chronic fatigue, joint and muscle pain and poor elimination of toxins.

4. The brain thrives on Omega-3. Without enough, we can experience learning problems, memory issues, brain fog, ADHD symptoms and other neurological symptoms. Proper levels help protect us from Alzheimer’s Disease. Balanced omega fatty acids are crucial in cardiovascular health and healthy blood pressure maintenance and insulin and blood sugar balance.

5. Food is a great source of Omega-3. Consider fatty fish like salmon, tuna, halibut, krill, flaxseed, walnuts and chia seeds (the latter 3 however give us ALA which needs to be converted to DHA & EPA).

6. It’s difficult to get sufficient amounts from food alone. Most Americans consume a daily average of 130 mg EPA + DHA – way below the recommended 1000-2000 mg. Consider adding a supplement to your diet. The Omega 3 supplement my family and I use, and I recommend to patients in my practice is International Fish Oil Standards (IFOS) five-star certified, which assures the highest level of purity, stability, and potency in fish oils. Each dose of this concentrated fish oil provides a whopping 900 mg of EPA and DHA in one softgel. Always check the the EPA and DHA content at the back of your bottle, as many times manufacturers will list a certain mg of fish oil on the front label of the fish oil bottle and it does not provide the accurate amounts of EPA and DHA that each serving has. This can mislead the buyer. 

7. Acquiring Omega-3 must be done in a focused fashion, with attention paid to the balance between Omega-3 and Omega-6 (a group of fatty acids linked to increased inflammation). An imbalance can adversely impact well being and brain health. 

8. Not all Omega 6s are bad! One known as DGLA (Dihomo-gamma-Linoleic Acid) is an antiinflammatory Omega 6 fatty acid. Other ones known as LA (Linoleic Acid) and AA (Arachindonic Acid) can be inflammatory in high amounts or when not in balanced amounts compared with DGLA. 

In my testing I look at individual levels of these key Omega 3 and 6 Fatty Acids but also the ratio of Linoleic to DGLA as seen in the image above. A simple blood test can give me all of the above information.

Interesting fact: The earliest known written record that likely referred to diabetes was in 1500 B.C in the Egyptian Ebers papyrus. It referred to the symptoms of frequent urination.

Basically, diabetes is a disease in which the body experiences elevated levels of blood sugar (glucose) due to an inability to either produce or use insulin. Most of the food we eat is turned into glucose, which our body needs for energy. In response to the rise in blood glucose, the pancreas makes a hormone called insulin, to help move the glucose into our cells for an ongoing source of energy. When you have diabetes, the body either doesn’t make enough insulin (Type-1 DM) or can’t use its own insulin efficiently (Type-2 DM). This causes glucose to build up in the blood, creating a potentially dangerous situation.

Type-1 DM is a chronic auto-immune health condition in which the immune system ravages the insulin-producing cells of the pancreas, causing a loss of the hormone insulin and affecting the way glucose is metabolized. Because of the loss of insulin, the body cannot move glucose from the blood into the cells where it is needed. Instead, glucose levels run high in the blood causing system-wide damage. While Integrative holistic health approaches can support the body, there is no cure once the pancreas are completely damaged and life-long management REQUIRES insulin. As a functional medicine doctor I have see halting of the disease process and even reversal if on early testing of the patient, the anti-bodies specific for type 1 diabetes are picked up and appropriate measures instituted. Type-2 DM develops from lifestyle choices. A highly preventable disease, it was once most common in middle-aged and older people. Today, it strikes an alarming number of young adults and children. It’s directly related to poor eating and exercise habits, which typically results in being overweight – a risk factor for Type-2 DM. In this type of diabetes, your body produces insulin but does not recognize and use it properly. If health is not restored through diet, lifestyle changes, and holistic approaches, Type-2 DM can progress to a state in which insulin is required.

insulin resistance & Pre-diabetes are  your warning signs, conditions, respectively in which your insulin levels are high but the body cannot utilize it appropriately and the blood glucose level is chronically above normal, but not yet high enough to be diagnosed as Type-2 DM. These stages is your chance to stop the onset of diabetes in its tracks by improving your lifestyle choices.

 

A few simple guidelines can help you manage diabetes, and even prevent Type-2 DM:

  • Eat fresh whole foods, drink plenty of water, increase dietary fiber and the amount of dark fruits and veggies in your daily diet. Avoid processed foods and added sugars. Eat what a plant makes, not what is made in a plant!
  • Exercise 30 minutes per day everyday.
  • Supplement with a good high quality  multivitamin/mineral, pure Omega-3 Essential Fatty Acids and B- complex vitamin with activated bioavailable forms of B vitamins.
  • Test your vitamin D levels and make sure they are optimal. Take supplementation if necessary to raise levels.
  • To get higher support for blood sugars try a supplement formulation specially created for that purpose and a meal replacement shake which has ingredients that support a healthy blood sugar and weight.
  • Try a safe and guided detoxification protocol a few times a year especially in the spring and the fall to give a “reboot” service to your body.
  • Consult with a registered dietitian with functional medicine training or a Health Coach to learn how to plan and prepare healthy meals.
  • Ask your Functional medicine doctor about food allergy and sensitivity testing; nutritional deficiency testing; and comprehensive gut function testing. Also check into your adrenal health as high cortisol levels can lead to or exaggerate your condition.
  • Make sure you’re not constipated. If so increase fiber in your diet, and if needed try safe supportive supplements. Daily bowel movement(s) is imperative to good health!
  • Take high quality multi-species probiotic supplements in the capsule or powder forms. These come in refrigerated or shelf stable forms. I love to use the latter during my travels!
  • Keep your skin healthy (hydration and whole foods). Use non-toxic personal care products
  • Use natural remedies such as herbal supplements, vitamins, detoxification, and dietary adjustments under the supervision of a Board certified Integrative Medicine (ABOIM) physician and Functional Medicine trained physician (IFMCP). Try a safe and guided detoxification protocol a few times a year.
  • Take medications or supplements as directed by your primary care doctor or endocrinologist.
  • Take particular care of your feet. Carefully monitor wounds, because many people with DM experience poor circulation and neuropathy. Vitamin C and zinc can help support would healing
  • Don’t forget to take care of your mind and spirit! Emotions, stress, lack of sleep all affect the metabolic functioning of the body. Practice mindfulness of some form daily!

If you’ve been diagnosed with Diabetes mellitus (DM), or even pre-diabetes and Insulin resistance, don’t take it lightly. Follow treatment plans and lifestyle recommendations strictly. Left untreated, diabetes can lead to many complications such as heart disease, blindness, kidney failure, and lower-extremity amputations. It’s the seventh leading cause of death in the United States.

Coming from an Indian heritage, lentils have been an important part of my daily meals all my life. I absolutely love them and really wanted to share their beneficial properties with all of you readers and foodies!

Around the world, people enjoy the health benefits of lentils, part of a group of proteins known as pulses, which also includes beans, peas, chickpeas. Naturally gluten-free, lentils are rich in dietary fiber, protein, calcium, potassium, zinc, and iron. They help support lower cholesterol levels and are a great addition to the diet especially for people diagnosed with blood glucose/ blood sugar disorders.

Prior to the use of pharmaceutical medicines, lentils were used to deal with diabetic conditions. When included with a meal, the high fiber content helps prevent blood glucose from rising rapidly after eating. Although calorie dense (230 cal/ one cup serving), lentils are low in fat and very filling – you won’t be hungry after a lentil meal! I can vouch for that one from extended personal experience!

You can find lentils in the bulk bin aisle or in prepackaged containers.

A few tips on purchasing and cooking:

When purchasing in bulk, try to buy organic and make sure there is no moisture in the bin or in the packaging. Look for whole, not cracked lentils. Store them in an airtight container in a cool, dark and dry place. They will keep up to a year. When buying canned lentils, watch for added salt or other preservatives. though I don’t recommend canned food AT ALL, just for your information, unlike other canned veggies, lentils do not lose much of their nutritional potency.

Lentils are easy to prepare (I do like to pre-soak them but if in a hurry, can cook without presoaking unlike other dry beans). Wash and strain lentils under cool water before cooking. You can boil lentils (I like to use my pressure cooker as a matter of habit, but you can use an Instant Pot as well if you have one) and store in the fridge for later use in casseroles, soups, rice or pasta dishes, salads, spreads/hummus, or soups. Cooked lentils stay fresh in the fridge in a covered container for about three days.

A cousin of watermelon, cucumber and pumpkin, Bitter Melon is shaped like a cucumber, only larger with lighter green and more gourd-like skin. In tropical cultures, where it’s cultivated, bitter melon is used to support digestion because of its ability to break down carbohydrates. When using for medicinal purposes, the entire plant can be used, dried or fresh, from leaves and stems to the actual juice.

Holistic physicians and researchers are interested in Bitter Melon for its effect on blood glucose levels in patients with diabetes mellitus. Chemicals in the extract act similar to insulin. A number of studies have found that bitter melon juice, fruit and dried powder have a moderate effect on lowering blood glucose. In other studies, a “plant insulin” injection given to patients with Type-1 diabetes showed a decrease in blood glucose. The decrease was not as significant for patients with Type-2 diabetes, but there was a decline in blood glucose levels compared to a control group. It seems that source and type of preparation, as well as individual patient factors, may play a role in the effect of bitter melon on diabetes, which will inspire further research.

In India, bitter melon is used to make different vegetable preparations. Juicing is another way to utilize its beneficial properties but since it is very bitter, I like to use some raw honey, beets, carrots and green apple with it. You can also blend bitter melon into various foods and enjoy it several times a week when it is in season.

Although considered relatively safe, consult your holistic practitioner to determine the appropriate type and amount to use for your particular needs. If you are pregnant or nursing, only use the supplement under the care of a qualified practitioner.

Hawthorn (Crataegus oxyacantha)

Hawthorn, also known as Maybush, is a thorny shrub found on hillsides and in sunlit woodsey areas throughout the world. Over centuries, all parts of the plant have been used to prepare foods, beverages, and medicines. In folk medicine, Hawthorn was used for the treatment of diarrhea, insomnia, and asthma. In China, it has been used to treat digestive problems, high cholesterol, poor circulation, and shortness of breath. During the early 1800s, doctors in North America used Hawthorn for heart conditions, circulatory, and respiratory disorders.

Hawthorn has a rich supply of flavonoids (antioxidants that protect cells from damage) and anti-inflammatory properties, which are important to heart health. It plays a role in helping dilate blood vessels, improves blood flow to the heart, and can lower blood pressure. In Europe, Hawthorn is regarded as a safe and effective treatment for early-stage heart disease. It is used to promote the health of the circulatory system and in patients with angina, high blood pressure, and congestive heart failure. In studies, patients with heart failure who took Hawthorn showed improvement in clinical symptoms and sense of wellbeing.

Mighty Pomegranate (Punica granatum)

Inside a lumpy, thick-skinned pomegranate you’ll find a treasure of jewel-like arils-ruby seeds surrounded by sacs of flavorful juice. Pomegranate is both richly sweet and tart and exceptionally refreshing. Savor the flavor by eating the arils by the spoonful, adding them to salads or to a bowl of Greek yogurt. Any way you choose, you’ll reap the health benefits of this exotic fruit.

Pomegranate is native to the mountainous regions along the Caspian Sea, near northern Iraq and northwest Iran. For centuries, cultures around the world have used all parts of the tree—roots, bark, flowers, peel, seed and seed oil—medicinally to treat a range of health concerns, from digestive disorders and dysentery, to fever and heart ailments. In the West, the fruit and seed are typically used in medicinal preparations. Modern research indicates that pomegranate may be beneficial for reducing risk and supporting treatment for arthritis, certain types of cancers, erectile dysfunction, and heart disease.

A compound found only in pomegranates called punicalagin is beneficial to the heart and blood vessels. Punicalagin is responsible for pomegranate’s antioxidant effects. In fact, pomegranate has more antioxidant power than red wine and green tea! Preliminary research shows that drinking unsweetened pomegranate juice helps lower cholesterol, lower blood pressure, improve blood flow to the heart, and may help protect against the formation of blockages in the arteries. Further research is needed to determine how much pomegranate juice (or as a nutritional supplement) is beneficial for different people and for different health concerns.

When buying pomegranate, don’t shy away from a fruit that isn’t perfectly round or feels heavy. Unusual shape, and weight indicate a fruit that is plump with arils and juice. Pomegranates do not sweeten once picked, so you’ll want to avoid fruits that look dried out.

Heart stethoscopeLong before the ancient Greek surgeon Galen carried out meticulous dissections of the heart, the Egyptians wrote about health and disease in relation to how the heart “speaks in vessels” with the rest of the body. Today, physicians may not associate the heart with the soul (or soul mates), but many credit early Egyptian medical knowledge of the heart as a precursor to modern cardiology.

A key element of a healthy body is a healthy heart. The heart is the center of our cardiovascular system and beats an average of 100,000 times per day supplying oxygen rich blood to the whole body. Every day we make choices that have a profound affect on the health of this vital organ. Most heart disease (HD) is linked to inflammatory risk factors such as lack of exercise, obesity, smoking, stress, and poor eating habits.

One major condition that can develop with these risk factors is Hypertension, also known as high blood pressure. Often called the ‘silent killer’, Hypertension can cause significant damage throughout the cardiovascular and other body systems and ultimately results in over 80 million deaths each year.

The Silent Killer

Blood pressure is the amount of pressure exerted on the inside of blood vessels as the heart pumps the blood through the body. When there is resistance in the vessels, the pressure rises and hypertension results. The longer hypertension goes undetected and/or uncontrolled, the greater the damage to blood vessels and other organs. Hypertension can lead to heart attack, stroke, ruptured blood vessels, kidney disease or failure, and heart failure.

Warning signs for high blood pressure are rare but can include headaches, blurred vision, light-headedness, shortness of breath and nosebleeds. However, there are typically no warning signs or symptoms for hypertension, which is why it is called the silent killer.

Hypertension is diagnosed by looking at 2 numbers in your BP reading: Systolic pressure (the top number) is the pressure in your arteries when the heart beats (contracts). Diastolic pressure (bottom number) represents the pressure in your arteries between beats.

• Normal blood pressure is below 120/80
• Prehypertension is 120 – 139 systolic or 80 – 89 diastolic.
• Hypertension is 140/90 or higher

The Potassium Secret for a Healthy Heart
You’ve no doubt heard the best thing to do when you have hypertension is to reduce the amount of salt/sodium in your diet. Did you know the average adult needs 4,700 mg of potassium daily compared to only 200 mg of sodium. Unfortunately for most of us, our eating habits give us way too much sodium – 3,300 mg a day – and not nearly enough potassium. This imbalance can increase your risk of developing hypertension.

What’s truly important for your heart, and a more accurate strategy to prevent high blood pressure, is to balance the relationship between potassium and sodium (salt) in your daily diet. Proper sodium-potassium balance is necessary for nerve transmission, muscle contraction, fluid balance, and the optimal health of all the cells in your body. In regard to the heart, potassium is particularly important for regulating heart rhythm and maintaining blood pressure.

By reducing your sodium intake, you are often correcting the sodium-potassium imbalance without realizing it. To further support your heart health, eat more potassium-rich foods such as sweet potato, spinach, banana, peas, legumes, apricots, avocados, halibut and molasses.

More Healthy Heart Tips & Heart-Healthy Diet Do’s:

  • Eat a variety of fresh fruits and dark green veggies daily.
  • Use plant-based oils for cooking.
  • Eat mindfully, not on-the-run.
  • Reduce or eliminate packaged foods, sugar, and red meat.
  • Walk, No Need to Run: 30 minutes of daily, brisk walking lowers your risk for hypertension.
  • Be Calm: Learn to manage stress with healthy coping techniques, such as, deep breathing, yoga, meditation, gratitude journaling, and getting quality sleep.
  • Supplemental Support: Nutritional supplements shown to support heart health include Hawthorn, CoQ10, Essential Fatty Acids (Omega 3), Magnesium, Garlic and B-vitamins. Karva Chauth Karva Chauth: some tips on the Indian traditional day of fasting, love & rejuvenation

Karva Chauth is an ancient tradition practiced by Hindu women especially in Northern India. During this day which involves a fasting ritual, the woman prays for her husband’s good health and long life. It is a day of celebration of love and togetherness, and of gratitude for all the blessings.

After fasting for the entire day, women dress up in colorful traditional clothing and jewelry in the evenings and pray together in a group before sunset, but eating only after sighting the moon in the presence of their husbands in the night.

Karva Chauth: some tips on the Indian traditional day of fasting, love, celebration & rejuvenation

The visualization of the moon is a big, but fun challenge for all in the household and community. The mischievous moon is most evasive that night!

This day of fasting and prayer incidentally coincides with the change of the season and the cooling of the weather outside. It is an excellent practice as it detoxifies the body as well as the mind, and what better time to do it than at the change of the season. 

Biologically speaking, women are practicing intermittent fasting in this tradition. This allows the body, but especially the gastrointestinal system (including the liver) time to rest and release toxic material (Ama, a term for toxins in Ayurveda), and stimulates body organs to rejuvenate and function better. Do remember that during the fast, there is release of a variety of toxins from the body tissues and this can lead to symptoms of aching, fatigue, brain fog, headache and irritability.

It is better to prepare for the day of fasting by starting to clean up your eating habits from a week before the day of the actual fast. This includes giving up alcohol intake and reducing caffeine. Being prepared will result in less adverse symptoms during the actual fasting period.

 

A few words of caution:

  • If you have an illness like cancer, heart disease, kidney disease or a  current infection, fasting may or may not be appropriate for you, so always seek medical advice for your unique situation.
  • If you are a diabetic, and on either oral medication or insulin for it, please make sure you eat intermittently and/or change your medication schedule on the day of fasting. 
  • If you are pregnant please do make sure you are consuming fresh fruits and veggies and water or lemonade during the day.

Always consult with you physician whether it is advisable for you to fast at all.

 

Tips for the morning of the Karvachauth fast:

Traditionally women eat Sargi (a snack) after prayers and before sunrise on the day of the fast. Try to eat protein rich foods, and not eat a high sugar or carbohydrate laden meal. The latter will cause blood sugar fluctuations during the day which will make you more hungry and irritable. Protein, on the other hand, stabilizes mood and blood sugar keeping you full longer.

Nuts are a great option as they not only contain protein but also have excellent essential fats which nurture the brain and body throughout the day.

Make sure to hydrate very well in the morning especially if you follow a strict “no-water” fast!

 

The fun part:

Remember fasting is not just a ritual for the body, it is also rejuvenation for the mind and our opportunity to return to the wholeness of our souls. It is your time to relax & regenerate! So make the most of it!

Karva Chauth: some tips on the Indian traditional day of fasting, love, relaxation, & rejuvenation

 

A few tips for relaxation & rejuvenation:

  • Listen to soothing music,
  • Practice meditation or mindfulness,
  • Try pranayama (breathing techniques),
  • Try restorative yoga techniques or corpse pose (Shavasana) with the intention to heal the body, mind and soul,
  • During the day if you have the luxury to be off work (or at least take half a day off!), try to pamper your self with a spa massage or at least do a self-massage (self -abhyanga)
  • Don’t over-exert physically or mentally. Read your favorite book.
  • If you’re ok with consuming water during the fast, sip some chamomile tea or other herbal teas
  • Take a warm bath with a few drops of your favorite essential oils, especially if you feel fatigue, headache, or body aches.
  • If nothing else, take a nap or just go see a movie with your friends.

Celebrate YOU!

This is your chance!

 

Tip on breaking your fast:

After the evening puja (prayer typically done with a gathering of friends), start with a glass or two of room temperature or luke-warm water. You can even add a little lemon, himalayan salt, and honey to the water. Natural, unsweetened coconut water is also a great option for slowly introducing electrolytes back into the body.

I find most women craving for tea or coffee, but those probably are not good ideas as they can cause gastric distress and even nausea.

I would also not recommend consuming any ice cold fluids.

Typically the moon is notorious for playing hide and seek within the clouds on the day of Karva Chauth! Once you’re fortunate enough to having a viewing (hopefully an early one!), follow these tips to introduce food after your fast in a healthy way:

After the moon is out and finally the fast is broken, please venture slowly into eating again.

First add in some fresh fruits in small amounts. Then after a little while have your dinner.

Fresh home made yogurt or coconut kefir (if you are dairy sensitive) are great as they help in the digestion of nutrients which the body greatly needs after a full day of starvation.

Moog lentil soup (daal) with a little white basmati rice and ghee are great options along with some easy to digest well cooked vegetables.

Eating fried and oily foods  may be counterproductive as those are harder to digest and our stomach’s digestive fire (agni, an Ayuvedic term) as well as our pancreas may not be ready for such heavy foods.

Finally enjoy and celebrate your time with family and friends!

Seems like everyone’s talking about inflammation these days! More and more studies are coming out linking chronic inflammation to a vast majority of chronic diseases, even cancer.
But, is all inflammation bad?

Not at all! Some inflammation is absolutely necessary for our survival. It helps us overcome any acute insult or injury to the body. For example, if you cut your finger, cells are recruited to the wounded area, and through a very complex mechanism, the body heals itself quite efficiently. That’s an example of inflammation working to heal you.

Similarly, if you contract a bacterial infection, the body quickly produces inflammatory chemicals known as cytokines, which help kill the foreign invaders. During this fight between the bugs and your immune system, you feel terrible, but only for a short time. Soon, you’re up and on your merry way again with little long-term botheration from the incident.
It’s only when this inflammation becomes chronic or long term—like a low-grade smoldering fire which does not die out—that it leads to major problems.
Normally, during a period of low-grade inflammation, the body tries to warn us that something is wrong. It does this by producing many nagging signs and symptoms, like:

  • Aching of joints and muscles
  • High blood pressure
  • Rising blood sugars
  • Headaches
  • Sleep issues
  • Allergies
  • Skin issues
  • Frequent abdominal pain and other gut issues
  • Mood swings
  • Difficulty with sleep
  • Low energy levels

If we do not heed these symptoms and allow the inflammation to persist unchecked, after years (yes, years!), the inflammation becomes evident in the form of a heart attack, stroke, Alzheimer’s disease, cancer, or an autoimmune disease like multiple sclerosis, ulcerative colitis or rheumatoid arthritis, which can no longer be ignored by us.
Some signs of chronic inflammation are:

  1. Chronic fatigue and low energy.
  2. Mood issues including anxiety, depression, and panic attacks.
  3. Memory problems, brain fog, or loss of focus.
  4. Difficulty falling asleep, waking up in the middle of the night, or unrestful sleep.
  5. Gut symptoms including acid reflux, IBS, constipation, diarrhea, blood or mucus in stools, intermittent abdominal pain, gas and bloating.
  6. Chronic muscle and joint pain, swelling, redness.
  7. Chronic allergies and asthma requiring frequent medications.
  8. Chronic skin issues like eczema, psoriasis, redness of face and cheeks, or hives.
  9. Frequent or daily headaches, tremors, easy loss of balance, tingling and pain along nerves in the extremities, dizziness.
  10. High blood pressure.
  11. Metabolic syndrome, abdominal obesity, or diabetes mellitus.
  12. Low bone density or osteoporosis.
  13. Heart disease, peripheral vascular disease.
  14. Diagnosis of cancer: Yes, taking action is possible even after being diagnosed with a cancer as you can still try to prevent future development of new cancers at other sites.

What should you do if you have any any signs of chronic inflammation? 
1. Find a functional medicine physician who is well versed in evaluating your symptoms. 
This person can help you eliminate the root cause of the chronic inflammation, rather than focusing on just suppressing symptoms.
2. Avoid all inflammatory foods. 
This includes refined sugars, sugar substitutes, juices and sodas, trans-fats, dairy products, non-organic eggs and meats from grain fed animals. I also suggested you eliminate any foods to which you may be sensitive/ allergic, all GMO foods, and foods with a high glycemic index.
3. Eat real foods. 
Incorporate large amounts of antioxidants, phytochemicals, and fiber-rich fruits and vegetables, nuts and seeds, small amounts of grass fed meats and wild small fish, healthy spices like turmeric, ginger, garlic, and herbs like cilantro, parsley, oregano and rosemary in your diet.
4. Stop eating microwavable lunches or dinners.
Don’t eat dead packaged and processed foods which have no life force!
5. Sip green tea on a regular basis. 
It cuts inflammation and calms the mind!
6. Try to avoid toxins as much as possible in your daily life. 
You’ll give your liver a much-needed break.
7. Incorporate a daily exercise regimen. 
Do yoga, tai chi or some other form of movement exercise. Move the body whenever you get a chance!
8. Meditate daily and breathe deeply. 
Who doesn’t love peace of mind? It’s also great for your nervous system.
9. Focus on optimal sleep. 
Your body restores and regenerates itself daily when you sleep!
10. Find time for rest and rejuvenation on a regular basis. 
Try to make time for mini-vacations and spending time in nature!
11. Get regular massages.
Better yet: learn the technique of self-massage and practice it daily!
12. Get your vitamin D levels checked and maintain optimal levels. 
Use supplements if needed. Vitamin D is not just a vitamin, it functions as a hormone messenger molecule in crucial body processes!
13. Maintain a positive mental attitude about life and people! 
Remember, just like your gut affects the brain, the brain in turn affects the gut, where the majority of our neurotransmitters are made.
Cutting down your risk of inflammation will most certainly reduce your chances of developing chronic diseases, so I urge you to take corrective action, one baby step at a time.

integrative medicineYou must have heard that “we are what we eat”, but are we really just that? Actually “we are what we eat (or drink), breathe, breakdown, digest, absorb (from our gut and skin), and cannot eliminate from our body, whether it’s through our thoughts, skin, breath, urine and stool! Even though we are designed to have built-in detoxification and elimination pathways, they can get overwhelmed with years of toxin exposures which our bodies are now having to handle more so than ever before in our evolution history.
My job as a Functional Medicine physician involves around dealing mainly with chronic ailments. I usually have to be a medical detective trying to help patients who have been through the conventional medical system of diagnosis and treatment of a “disease” and have found no relief.
Below is a quick check list that I keep in my mind of patients who might have signs of an overburdened detox system.
Those with:
1. Multiple Chemical Sensitivities (MCS):These patients react to just about everything- medications, nutritional supplements, foods, odors and chemicals in the environment!
2. Chronic neuropsychiatric conditions: Depression, Anxiety, Seasonal Affective disorders, Mood swings and refractory mood disorders, Chronic recurrent Headaches/ Migraines, ADHD, Learning Disorders, Autism Spectrum Disorders,
3. Neurodegenerative diseases: Parkinson’s disease, ALSAlzheimer’s dementia, some forms of peripheral neuropathies, Multiple
4. Mitochondrial disorders: Chronic Fatigue Syndrome, Fibromyalgia. Neurodegenerative disorders are also due to disturbances in mitochondrial energy production.
5. Immune system imbalance symptoms: frequent and chronic infections, allergies, asthma, eczema and other chronic dermatitis, autoimmune diseases (Hashimoto’s thyroiditis, Grave’s Disease, Rheumatoid Arthritis, Ulcerative Colitis to name a few), multiple food sensitivities, Dysbiosis (imbalance of gut bugs with possible Yeast Overgrowth or Small Intestinal Bacterial Overgrowth).
6. Signs of endocrine disruption: male and female infertility, precocious puberty, PCOS, PMS, fibroids, chronic pelvic pain, endometriosis, adrenal fatigue, hypothyroidism.
7.  Metabolic Conditions: overweight, obesity, hypoglycemia, hyperglycemia, Insulin Resistance, Diabetes Mellitus Type 2.
9.  Cancers
10. Chronic isolated low white cell counts (Leucopenia) or low red cell counts (Anemia) without any known associated cause.
You are probably aware that Functional Medicine practitioners usually take very detailed histories, do detailed examinations, and may do extensive specialized lab (Biomarker) testing. Not every test in the list below usually needs to be done on every patient. Based on clinical evaluation, the need for testing as well as the interventions are very individualized. For example, no two patients with type 2 diabetes or depression will have the same cause for it and thus testing and interventions will vary widely.

While the list below is by no means exhaustive, some general evaluation guidelines are:

1. Detailed history and intake with the help of Medical Symptom Questionnaires, Toxicity and Exposure questionnaires, Environmental and Occupational Exposure Questionnaires.

2.  Detailed Nutritional, Dental and Physical examination.

3.  Conventional Lab tests:  can be done from local labs like Labcorp/ Quest etc

Complete Blood Count looking for low white cell count, anemia, low patelet count.

Complete Metabolic Panel looking at liver enzymes, bilirubin levels, alkaline phosphatase, gamma glutamyl transferase levels.

Kidney Function Tests: Blood urea nitrogen, creatinine, cystatin C

Metabolic testing: Fasting and 2 hour post prandial insulin and blood sugar testing, fructosamine, hemoglobin A1C, detailed lipoprotein profile to assess for LDL and HDL particle numbers and sizes and other markers of inflammation specific to the vascular system

Indices like C-reactive protein, ferritin, fibrinogen to evaluate presence of inflammation.

Detailed thyroid function tests along with evaluation of antibodies against thyroid gland.

Iron absorption and storage assessment

4.  Functional Labs to test for Biomarkers: These can be:

Stool Assessments: to look for growth of disease causing bacteria/yeast in the gut, and also to assess the balance of good vs bad bacteria in the gut; looking for parasites; markers of inflammation of the gut; markers of deficient detoxifications ability in the gut itself (which can lead to recirculation within our body of our own estrogens, cancer promoters and hormone mimickers from the environment, instead of them being eliminated in the stool); assessment of pancreatic enzymes production; looking for markers of healthy absorption vs malabsorption.

Assessment for nutritional deficiencies (of all fat and water soluble vitamins, minerals, essential amino acids, antioxidants {including the master antioxidant, glutathione}, essential fatty acids). Nutritional support is a key factor influencing the proper functioning of the different phases of detoxification as I have mentioned in my previous blog

Serum testing for food sensitivities (IgG), and environmental and food allergies (IgE).

Specialized genetic and antibody testing for Gluten as well as cross-reactive foods antibodies.

Saliva testing for male and female hormones and adrenal health. If you are placed on Bio-identical hormones, you must periodically also get testeded to see how the hormones are metabolized in your body, which can vary widely. If not adequately metabolized and detoxified these can increase the risk of cancer (like breast, prostate and other hormonally related cancers) development.

Testing for neurotransmitter levels, as well as balance/imbalance between the excitatory and inhibitory neurotransmitters. This can be helpful in patients with anxiety, depression, attention problems or chronic sleep issues.

Looking for heavy metal toxic burden on the body, if indicated by a detailed clinical evaluation. One example of this would be if you have silver mercury amalgam(s) in your teeth especially old ones.

Assessment for signs of oxidative stress or damage (similar to rusting of iron that occurs in nature on exposure to oxygen) to the body due to toxins: these tests look for markers of DNA damage, as well as damage to circulating fats (lipids) in the blood. Damage (aka oxidation) to LDL, the bad cholesterol, may lead to coronary artery disease.

If indicated, checking for chronic stealth infections due to bacteria, parasites and viruses which may harm the immune system and other organ systems. These could present as chronic fatigue, muscle and joint pains, autoimmune diseases and many other vague nagging symptoms.

If indicated, evaluating and testing for mold exposure and mycotoxins. You may need to find a physician who has experience in diagnosing and treating mold related illnesses.

Genomics Testing: to assess for genetic variations known as SNPs (Single Nucleotide Polymorphisms) – like the commonly known MTHFR variants among so many others – which may lead to an increased genetic susceptibility towards accumulating toxins  and limit the detoxification capacity of our phase 1 and/or phase 2 liver enzymes.

Most of the above testing can be ordered by any physician, but the interpretation (which really is the key!) can probably only be done accurately by a trained Functional Medicine (FM) Practitioner. I strongly feel that someone with a chronic health issue, should start with a FM doctor as soon as possible, while fully realizing that in some parts of the country and many parts of the world this is not an option. Also, there are some FM practitioners who are not medical doctors and may not be able to order some of these tests. In those cases the tests could be ordered by your regular MD, with results being preferably interpreted by your trained FM Practitioner.

Most people have their blood pressure, heart rate, lipid profile, fasting blood sugar, and even vitamin D levels measured during their yearly physical. Your physician may do an EKG or a stress test. All of these tests may be “normal,” but there are other significant risk factors for metabolic issues that contribute to cardiovascular disease, that may be completely missed unless more testing is done.
Below is a list of some easily run lab tests and calculations which give insight into risk for insulin resistance, metabolic syndrome, inflammation and risk for future coronary heart disease. In my opinion, these tests should most certainly be a part of any patient’s assessment for metabolic disorders and development of heart disease:

1. Triglyceride to HDL ratio

This is a simple measure of insulin resistance, risk for diabetes mellitus type 2 and cardiovascular disease. A ratio less than 3.0 is considered normal, and more than or equal to 3.0 is suggestive of insulin resistance. An elevated ratio should prompt counseling the patient about therapeutic lifestyle change with close follow-up of metabolic markers for prevention of development of diabetes, hypertension and coronary artery disease.

2. Fasting insulin levels (and, if possible, two-hour postprandial insulin levels)

Usually at the yearly physical your physician will measure fasting blood sugar, and may even measure hemoglobin A1c levels, which are markers for pre-diabetes and diabetes. In my opinion, abnormalities in these are late developments. Insulin is a hormone produced by cells in the pancreas; it’s responsible for regulating blood sugar levels and for promoting uptake of glucose into the cells for use as energy or for storage as fat. Measuring insulin levels two hours after a glucose load may be the first indication of metabolic problems in your handling of sugar (or carbs).
Sometimes a two-hour insulin measurement may be difficult, so in my practice I measure fasting insulin levels, which are the second measure to go up. These two can be early markers of insulin resistance, much before fasting blood sugar starts going up. Excessive insulin levels lead to increased serum triglyceride levels, decreased serum HDL (good cholesterol) levels, increase in levels of the small dense LDL (the plaque-promoting form of bad cholesterol) particles. Insulin resistance is a risk factor for hyperlipidemia, diabetes, hypertension, all of which contribute to atherosclerosis and plaque formation, which in turn is a risk factor for heart disease development. I encourage my patients to work toward a fasting insulin level of at least less than 6 microunits per mL (ideal is 4). Obviously diet is the key for reducing Insulin levels, but for patients who need additional short or long term support, I use a supplement known as Blood Sugar Support which have all the key factors to support blood sugar and insulin levels. It has helped many of my patients.

3. High-sensitivity C-reactive protein (hsCRP)

CRP is produced in the liver in response to nonspecific inflammation in the body. The high-sensitivity CRP is an assay which detects low levels of CRP specific to inflammation in the blood stream i.e., vascular inflammation, which could be the cause for development and progression of heart disease. Standard lipid tests do not test for inflammation, and this marker can be easily tested through your local labs. Even if your lipid profile is normal and you do not have other cardiovascular disease risk factors like hypertension or obesity or diabetes, if this marker is elevated, it is cause for concern. If somebody has already had a cardiac event like a heart attack or stroke, and this marker stays chronically elevated, it could be predictive of a high risk for future repeat heart attack or stroke. The therapeutic goal for hs-CRP is less than 0.7 mg/L, preferably as near to 0 as possible. I like to repeat this a few times to be sure it is actually staying high.
A word of caution with hs-CRP: though a marker of inflammation, it can also be a marker for infection and acute injury, so if you’ve had any recent surgery or procedure; have a respiratory, gum or other infection; or have the flu, please wait until your symptoms are resolved before testing, as this marker will be elevated due to that acute event.

4. Red blood cell magnesium levels

Red blood cell magnesium levels are the best way to assess magnesium status. Clinical signs of magnesium deficiency could be fatigue, weakness, muscle cramps, seizures, numbness and tingling in the extremities, mood and sleep issues, irregular heartbeats and also spasms in the arteries which feed the heart, contributing to heart attacks. Magnesium supplementation may reduce the risk of coronary heart disease, so try to keep levels at the high end of the lab range, though I monitor clinically more often. The form of magnesium I use is a very highly absorbable chelate.

5. Homocysteine levels

Homocysteine is a toxic amino acid and elevated levels can be a risk factor for development of cardiovascular disease and stroke. Elevated homocysteine levels damage LDL cholesterol, causing its oxidation. Oxidized LDL leads to plaque formation in the coronary arteries. Homocysteine is produced as an intermediate in the metabolism of amino acids methionine and cysteine. Activated forms of vitamin B12 and folate are needed for the conversion of homocysteine into methionine, so elevated homocysteine can be one of the indicators of deficiency in these important vitamins (remember that there are drugs like Methotrexate and acid blockers like Nexium which block the metabolism or absorption of these vitamins and may cause elevations in homocysteine levels). Serum homocysteine is great screening test for patients who are at a risk for cardiovascular disease and stroke, especially when there is a family history but no other known risk factors. Strive to keep levels around 8 micromoles/L. If elevated Homocysteine levels are due to a genetic variation in key enzymes of the methylation pathway then supplementation will usually be required long-term with frequent (every 3 months) monitoring of levels.

6. Ferritin levels

Ferritin is an iron-containing protein produced in the liver, and signifies the stored form of iron. Ferritin can also be a marker of inflammation. Moderate elevation of ferritin may lead to a 2-3 times increased risk for development of diabetes. Though the normal range in your lab for ferritin can be from 12-150 ng/mL in women and 12-300 ng/mL in males, studies have shown high ferritin levels increase cardiovascular risk. Lowering the iron stores to near optimal level of between 70-80ng/mL will improve outcomes in cardiovascular disease including reduced risk for heart attacks, strokes and also improve life expectancy. As a side note, I don’t think that ferritin levels less than 50ng/mL are in any way normal (even though your lab may show it in the normal range) and reasons for low iron stores must be looked into.

7. Testosterone levels (in males only — no correlation with heart disease has been found in women)

In males, testosterone deficiency has been associated with a high risk for cardiovascular disease. In elderly males, lower free testosterone level has been shown to be associated with a high prevalence of cardiovascular disease. If your total and free testosterone levels are low, work within integrative or a functional medicine physician to find out why that is the case. Replacing testosterone may not be the only answer. Certain natural supplements like Zinc, Magnesium, L-carnitine, B vitamins especially B6 can help support Testosterone levels naturally.
Males should try to keep testosterone levels between 350-600ng/dl. Remember, more is not always better with testosterone therapy if you need it, and always seek out a physician well versed in male hormonal replacement therapies, or someone who can help guide you to support your body’s hormone production naturally!

8. Testing for chronic infections

Many pathogenic bugs have been shown to have a significant correlation with development of coronary heart disease as well as acute coronary syndromes (heart attacks). Some of these are herpes simplex virus, cytomegalovirus, and H. pylori. Ask your physician to test for antibodies to these bugs through your local laboratories. Since most of your immune system is in the gut, taking a multistrain good quality probiotic can be a great first step.
Print out and take this list to your physician to help them better evaluate your risk for heart disease, or find a functional medicine physician who is well versed in managing and counseling patients with metabolic disorders!
